Health & Home Care brings nursing, rehabilitation and personal care services to your home. The Wesley Des Moines Health Center offers additional care options, including rehabilitation therapies, skilled nursing and memory care. Skilled Nursing and Rehabilitation

Choose the Wesley Des Moines Health Center when you need help recovering from surgery or an illness, long-term skilled nursing care or rehabilitative therapies. Registered and licensed practical nurses and nursing assistants provide care 24-hours a day under the supervision of the resident's physician. Our interdisciplinary team of professionals works for you and with you to reach your highest possible level of health and wellness in our Medicare and Medicaid-certified center.
