People who use Wesley Homes Home Health are less likely to have to return to the hospital. Medicare.gov, the official U.S. government site for Medicare, lists Wesley Homes Home Health at nine percent for patient re-admissions to the hospital. That’s well below the state and national averages of fifteen and sixteen percent respectively.
“We’ve been actively pursuing improvements,” said Tom Brown, Executive Director for Home Health. “These results tell us is that our communication efforts are paying off for our clients.”
Home Health Quality Improvement (HHQI) ranks Wesley Homes Home Health in the top 20 percent nationally.
“Most re-admits to the hospital occur within the first 10 days of discharge,” said Tom. “We focus on those days, making sure clients can manage their medications, increasing communication between the hospital and home health staff and making sure clients have appointments with their primary care physician (PCP), who may not have been the doctor to provide hospital treatment.
“We supply the PCP with a list of medications from the hospital and let him know our care plan,” continued Tom. “We also get the family and the patient engaged in the care protocol, an approach that improves outcomes.”
